Office
Hours
Our office hours are 9AM to 4PM EST. Monday through Friday. Be assured that
we provide emergency coverage 24 hours per day, 7 days a week.

Initial Visit
On your initial visit, please bring the following items:
Picture
Identification
Insurance card(s)
Written referral form, if required by your health plan
List of medications you are taking
Pertinent test results
Also, be prepared to share your medical history with Dr. Cornell. This
will include any medical conditions you have or may have had in the
past, surgical history, any medications (prescription or over-the-counter)
you are currently taking (include name, dosage and frequency), and any
allergies you have to medications or foods.

Charting Tool
This
Microsoft Excel worksheet allows charting of rising historical and future
PSA values by time period. Also there is a separate calculation for
doubling, usually in days. If the PSA is increasing very slowly the
user might want to use quarterly or annually.
