Office
Hours
Our office hours are 9AM to 4PM EST. Monday through Friday. Be assured that
we provide emergency coverage 24 hours per day, 7 days a week.
Initial Visit
On your initial visit, please bring the following items:
Picture
Identification
Insurance card(s)
Written referral form, if required by your health plan
List of medications you are taking
Pertinent test results
Also, be prepared to share your medical history with Dr. Cornell. This
will include any medical conditions you have or may have had in the
past, surgical history, any medications (prescription or over-the-counter)
you are currently taking (include name, dosage and frequency), and any
allergies you have to medications or foods.

We respect the fact that you are a busy person. In order to
keep your wait in our office to a minimum, we have put the following
forms online that you should download and complete, print off, and bring with you
to your initial office visit. If you are unable to print the forms, please arrive 15 minutes early to complete them in our office.

This
Microsoft Excel worksheet allows charting of rising historical and future
PSA values by time period. Also there is a separate calculation for
doubling, usually in days. If the PSA is increasing very slowly the
user might want to use quarterly or annually.

Our office is conveniently located in the Northside Professional Center:
993 Johnson Ferry Road NE~ Building C, Suite 225 ~ Atlanta, Georgia 30342
DRIVING DIRECTIONS- Our parking deck is accessed off of Meridian Mark Road.
If Heading East on I-285, take exit 26 (Glenridge, one exit past Roswell Rd) and turn right at the first light onto Glenridge. Make a left at the next light onto Johnson Ferry Road. At the second light (at the SunTrust Bank) make a right onto Meridian Mark Road. Turn right at the very next light into the Northside Professional Center. We are in building C on the left.
If Heading West on I-285, take exit 28 (Peachtree-Dunwoody Road). Turn left and go to Johnson Ferry Road (you will pass the MARTA station and St. Joseph’s Hospital on your left). Turn right onto Johnson Ferry Road. Turn left at the first traffic light onto Meridian Mark Road. Turn right at the very first light into the Northside Professional Center. We are in building C on the left.
If heading South on Georgia 400, pass the 285 exits and exit at the Glenridge Connector. Turn right at the end of the ramp; get into the left lane. Turn left at the very first traffic light onto Meridian Mark Road. At the next light, take a left into the Northside Professional Center. We are in building C on the left.
If heading North on Georgia 400, go through the toll (50¢) and take the next exit (4A) for Glenridge Connector/Peachtree Dunwoody Road/Johnson Ferry. Turn right at the end of the ramp; get into the left lane. Turn left at the very first traffic light onto Meridian Mark Road. At the next light, take a left into the Northside Professional Center. We are in building C on the left.
WALKING DIRECTIONS- If taking MARTA
Our office is within walking distance of the Medical Center Station.
Once outside of the MARTA station, walk up to the street (Peachtree-Dunwoody Road) and go left. At the second light make a right onto Johnson Ferry Road. At the very next light make a left onto Meridian Mark Road. You will see a SunTrust bank on the corner and we are directly behind it. Walk behind the SunTrust Bank and into the Northside Professional Center. Our office is located in building C, Suite 225. For a map of our area, please follow this link.
